If you are taking medication or have a long term condition like Diabetes, Asthma, COPD, hypertension, or a mental health illness, it requires us to work together to ensure that the medications are working and that you are taking control of your health.

We require your co-operation in monitoring and managing your medications and conditions.

However, if you do not meet with us, follow our advice, take control of your lifestyle, diet, exercise and medication; it will be difficult for us to help you the best we can.

Please make appointments for regular health reviews with our specialist nurses for long term conditions and doctors for mental health illnesses.

The most convenient way to manage appointments and repeat prescriptions is to sign up to online access. You will be able to book appointments, submit repeat prescriptions and view your medical record at any time of the day or night.
